Home Visit Information

• Please make sure any pets are secured in another room prior to your home visit. We are animal lovers, but for everyone’s safety, pets cannot be present during a home visit.

• We will need access to a sink for hand-washing, and a clean, flat surface (such as a table or desk) on which to place supplies.

• Payment is due in full at the time of service. Payment options include credit card, ACH transfer, and cash.

• If your child is not a Stowe Farm Pediatrics patient:

  • Documentation from your child’s visit will be faxed to their primary care physician within 48 hours of their home visit to ensure for continuity of care.

  • The home visit fee covers only the home visit service rendered that day, and basic follow-up care as needed (for example, notification of a send-out lab result). For more extensive follow-up care, a new visit must be scheduled, or care can be transferred back to your regular primary care physician or other service of your choice.

Once your home visit has been scheduled, you must pre-register your child with Stowe Farm Pediatrics prior to the visit so that a chart can be created. Payment information is also collected as part of registration.
